Certification Application Resource Page

Thank you for participating in the AACVPR Program Certification process. AACVPR has compiled the following resources to help you through the application process. The deadline to submit your application is Wednesday, February 29, 2012.

AACVPR Membership Requirement

To submit your certification application, you must have a current AACVPR member within your program. If you do not have a current member on staff or your membership is about to expire, please join AACVPR or renew your membership today.

Important Dates

• December 1, 2011: Certification period application open
• February 29, 2012: Certification period application closes
• March 1 – June 30, 2012: Application review period
• July 1–31, 2012: AACVPR Board of Directors reviews and approves recommendations for certification

Application Instructions

• Deadline: The deadline to submit your application is Wednesday, February 29, 2012, at 11:59pm ET.
• Membership Requirement: To submit your certification application, you must have a current AACVPR member within your program. Join AACVPR or renew your membership today.
• Click here to access the AACVPR Program Certification application.
• Log in with your AACVPR member user name and password.
  • Select NO when asked if your program has previously applied for certification using the AACVPR Online Certification Center. (Even if you are applying for recertification or are a 2010 or 2011 applicant that was not granted certification, please select NO. The Online Certification Center and the program intake form are new since your program last applied.)
• To ensure that you receive timely updates and critical information during the certification process, you must provide accurate contact information for your program. Please complete the program intake form as you would want your information to appear on a certificate.
• Tips for completing the program intake form:
  • Under Facility Name, click “Create New Facility.”
  • Add your facility name, address, and program name. PLEASE NOTE: You will need to complete a separate form for each cardiac and/or pulmonary rehabilitation program that is applying for certification.
  • Complete the date your program began. Your program must have been established on or before January 1, 2010.
  • Recertification applicants only: Indicate that your program has been previously certified.
    • Date of initial certification for Recertification applicants is August 31, 2009.
    • Certification expiration date for Recertification applicants is August 31, 2012.
  • The staff roster must include professional staff members (e.g., RN, RRT, exercise physiologist, exercise specialist, LPN, dietitian, P.T., certified respiratory therapist) if they are under the direct supervision of the CR/PR manager.
  • The staff roster also must include one for each of the following: administrator (CEO), medical director, program director, application primary contact, and application secondary contact.
    • The primary and secondary contacts are the only roles who can login and edit the application.  The primary contact must be a member.  Therefore, the member at the program has to login to start the process and then they can assign a secondary contact.  The secondary contact will receive an email with separate login information that will allow them access to the application.
    • It is important to input all relevant staff members into the staff roster to assist in evaluation of staff competencies.  This is a onetime entry; staff members will appear automatically for future certification cycles and you will only need to make updates.
