Award of Excellence:
The Award of Excellence is given each year in recognition of an outstanding contribution by an individual to the field of cardiopulmonary rehabilitation.
Criteria:
- Nominee should have made significant and unique professional contribution(s), which have had a major impact on cardiac and/or pulmonary rehabilitation.
- Nominee expertise will be recognized in the areas of clinical practice, research, or professional education, or a combination of these.
- Nominee should have earned a national prominence in his/her field as a result of contributions made over many years.

The Distinguished Service Awards are presented to individuals whose ideas, committee work and leadership have benefited the AACVPR as an organization in a significant way.
Criteria:
- Nominee has contributed ideas and volunteer leadership to benefit the AACVPR in a significant way.
- Nominee must be a current AACVPR member and have been active in the Association for a period of at least five years.
- Nominee must be currently active in a field related to cardiac and/or pulmonary rehabilitation.

L. Kent Smith Excellence in Clinical Practice Award:
In recognition of the vast and numerous scientific and clinical contributions made to the field of cardiovascular rehabilitation by the late L. Kent Smith, M.D., the Heart and Lung Foundation has established an award in his memory. The L. Kent Smith Excellence in Clinical Practice Award is designed to annually recognize one individual for his/her outstanding clinical contributions to the field of cardiopulmonary rehabilitation.
Criteria:
- Nominee should be a senior level professional in the field of cardiac and/or pulmonary rehabilitation.
- Nominee may or may not be a current member of AACVPR.
- Completed nomination form.
- Nomination letter presenting a compelling case for the nominee.
