Course Description
This presentation will review the process of patient referral and participation in cardiac and pulmonary rehabilitation through the lens of value-based care. During this session, attendees will learn how to and about reduction in health care utilization and cost, how to develop effective data tracking tools, and establish a rehab referral work flow.

Course Objectives
1. Understand the value of engaging health system Clinician and Administrative Leadership to support the cardiac          and pulmonary rehabilitation program.
2. Describe methods to identify eligible inpatients and facilitation of their referral to outpatient cardiac and             pulmonary rehabilitation programs.
3. Develop a data tracking mechanism (referral and enrollment, patient outcomes and costs) to support the growth  / development of the cardiac and pulmonary rehabilitation program.
