AACVPR Outpatient Cardiac Rehabilitation Data Registry

The AACVPR Outpatient Cardiac Rehabilitation Registry will be a unique and powerful tool for tracking patient outcomes and program performance in meeting evidence-based guidelines for secondary prevention of heart and vascular disease. It will provide cardiac rehabilitation programs with national outcomes data for benchmarking and demonstrate the positive impact of cardiac rehabilitation on the morbidity, mortality, physical function, and quality of life of heart patients across the United States.

Each participating program must establish a principal user for the registry; this is the main contact between AACVPR and the program.

  • If your program already has a principal registry user, you do not need to complete the Principal User Contact Form to continue the subscription process.
  • If you need to be set up as the principal user for your program, please complete the form at the bottom of this page and you will be sent next steps information via e-mail within three (3) business days.

If we already have you as a Principal User in our system, you may access your program information by logging in and clicking on "My Programs" in the right hand column of this page.

For renewing registry subscribers: Amendment to Participation Agreement (Word® doc)

In order to make the process as efficient as possible for you and for us, please do the following:

  • Insert the full and legal name of the entity that will be entering into the agreement on Page 12 where indicated;
  • In section 9.12, add the address where you would like notices sent, including the name of the person to whom the notice should be addressed;
  • If you have any suggested revisions, please make those revisions directly on the document in a way that clearly shows the changes (i.e. "track" changes in the Word document); and
  • Please do not make revisions and send us a PDF document. We cannot accept PDFs. Please send back a Word document with your tracked changes. 
  • Please note: All programs must use the provided Registry Participation Agreement, which includes a fully compliant HIPAA Business Associate Agreement and a HIPAA Data Use Agreement. Read more.

Influencing National Healthcare Practices

Outpatient cardiac rehabilitation (CR) is not well utilized in standard heart care, despite being recognized as a standard of care by the American College of Cardiology (ACC), American Hospital Association (AHA), American College of Chest Physicians (ACCP), and AACVPR. As healthcare systems begin to emphasize chronic disease management, we have a valuable opportunity to demonstrate the value of CR.

By tracking and providing comparative data for benchmarking of clinical, behavioral, health, and service outcomes, as well as tracking key program performance issues, the AACVPR Outpatient Cardiac Rehabilitation Registry will provide a solid base of scientific data to promote more widespread use of CR services for eligible heart patients.

The Benefits for CR Programs

Through the AACVPR Outpatient Cardiac Rehabilitation Registry, CR programs are able to:

  • Create and manage patient profiles
  • Enter and view outcomes data on enrolled patients in real time
  • Track and quantify progress of the CR program in meeting performance goals
  • Produce individual and grouped outcomes reports

By assisting in the collection, management, and interpretation of outcomes data, the registry will help CR programs:

  • Compare outcomes and processes to evidence-based goals and national benchmarks
  • Implement quality improvement projects based on real data
  • Enhance documentation and communication with the program’s key audiences, such as hospital administrators and referring physicians
  • Promote the role of CR in the management of chronic heart and vascular disease
  • Increase support from physicians
  • Improve 3rd-party payer coverage and reimbursement rates

Registry Functionality

Through this Web-based registry, subscribers can access accurate, high-quality data in real time. The data is secured using industry-standard SSL and rigorous encryption techniques.

Application programming interfaces (APIs) will allow CR programs with existing electronic systems to seamlessly and securely send data to the registry, while programs currently using a paper-based data collection system can step up to an easy-to-use interface that facilitates patient management and delivers critical patient care and program process feedback.

Data Collection and Reporting

Three categories of data, based on current practices and sound clinical evidence, are being collected:

  • Patient demographic information, such as age, ethnicity, and pre-existing comorbid conditions
  • Program performance measures such as referral rates, enrollment rates, completion rates, and medication compliance
  • Individual patient outcomes, including key clinical, psychosocial, and behavioral measures

Programs get immediate access to their patients’ data and will be able to see how their data compare to aggregated data from other programs in the registry. Reports based on various demographic variables will help programs determine whether there are inequalities in outcomes based on gender, ethnic, or age differences, or comorbidities.
