Registry Performance Measure Calculation Instructions and Clarifications for 2026 Program Certification Application
With the launch of the new Registry Platform, we have received a number of questions on how patients are included or excluded based on the performance measure calculations within the registry. After review and discussion with the Quality of Care Committee, please see below for clarification on these questions. Please make sure to refer to the full performance measure specifications for the complete list of requirements and exclusion criteria for each measure. These calculations are all live as described below as of November 1, 2025.

CR Functional Capacity, Blood Pressure, and Depression Measures:
Patients will be included or excluded from the measure calculations in the registry based on their DISCHARGE date. The discharge date must fall within the data collection period. The patient’s enrollment date and their referral date do not need to fall within the data collection period.
CR Tobacco Cessation Measure:
Patients will be included or excluded from the measure calculations in the registry based on their ENROLLMENT DATE. The enrollment date must fall within the data collection period. The patient’s referral date does not need to fall within the data collection period, as the measure as it is currently written only requires enrollment for inclusion. Discharge date is not assessed for this measure.
PR Functional Capacity, Dyspnea, and Health-Related Quality of Life Measures:
Patients will be included or excluded from the measure calculations in the registry based on their DISCHARGE date. The discharge date must fall within the data collection period. The patient’s enrollment date and their referral date do not need to fall within the data collection period.
CR & PR Enrollment Measure:
Patients will be included or excluded from the measure calculations in the registry based on their REFERRAL DATE. To qualify for the numerator of the measure, the patient must complete their index visit (ENROLLMENT DATE) within the data collection period. If the index visit takes place after the data collection period (i.e. starting in January 2026), they will be excluded from the calculations.
CR & PR Adherence Measure:
Patients will be included or excluded from the measure calculations based on their ENROLLMENT DATE, and DISCHARGE DATE – these dates need to be within the data collection period for them to be included in the calculations. Patients that enrolled prior to the data collection period or completed/dropped out of the program after the end of the data collection period will be excluded. To qualify for the numerator of the measure, PR patients must have completed at least 10 sessions, and CR patients must have completed at least 12 sessions.
